Changes to the directory do occur frequently. As a result, we are unable to guarantee that information in the directory is always current.
In order to assure the provider you choose is currently in your network when you need medical or dental care, it is important to contact your Provider Network to confirm provider participation.
Note: The Principal does not guarantee all services provided at a Preferred Provider Organization (PPO) hospital, PPO surgery center or PPO physician or dental office will be provided by a PPO provider. Services by a non-network provider, will be paid at non-PPO benefit levels. You may be responsible for payment of all or part of the fees for those professional services that are not paid or covered by your health benefit plan.
