Create a design to fit a wide range of budgets by selecting the service category (preventive, basic, and major), deductible, coinsurance, and maximum. Deductibles are waived when satisfied with prior carrier. When a group becomes effective, the annual maximum is refreshed. Use our mobile app for easy access to benefits, providers, and ID cards. Coverage is available to employees and dependents to age 26.

Minimum case size
  • Employer-paid: 2 enrolled employees
  •  Voluntary: 5 eligible employees
  •  Self-funded: 100 enrolled employees and at least one insured coverage

     
Participation requirements
  •  Employer-paid: 100% for non-contributory, or 50% for contributory or 2 enrolled employees (whichever is greater)
  •  Voluntary: 20% or 5 enrolled employees (whichever is greater)
